Something has been shot, if it was damaged by someone shooting at it. The glass in the picture has been shot, and has a bullet hole in it. Shot is the past tense of shoot.

Signed is the past tense of sign, but usually meaning signing your name as a signature. For example, "I signed for the parcel" or "I have a signed warrant".
